Folio 18. Letter from Messrs W and E Willoby [William Willoby and Edward Willoby], Clerks to the Guardians of the Berwick-upon-Tweed Poor Law Union, to Edwin Chadwick, [Secretary to the Poor Law Commission], referring to the circular of 2 January enclosing forms of return under the Vaccination Extension Act, requesting to be informed from whom they can obtain the information to complete the columns headed 'Number of Births in the District during the year' and 'number of deaths from small pox'. Annotated: 27 January 1845, that under the circumstances this information may be omitted from the form. Paper number: 1044/A/1845. Poor Law Union Number 325. Counties: Town and County of Berwick-upon-Tweed and Northumberland.
