Condition/Injury/Disease: Appendicitis (Old Operation); NYD [Not Yet Diagnosed] Abdominal.

Details and Outcome: Patient admitted for two periods of treatment in England suffering with symptoms from an old operation for appendicitis, and with an undiagnosed abdominal condition. During both courses of treatment patient underwent operations. Further details given. Transferred to [Horfield] 22 January 1919. Outcome not stated. Contains patient's home address. Includes two medical case sheets, an officers' form of particulars and a Medical Board medical report.
