Ref: 4366/2. A report of the primary total failures on B 0L.601 was first given, this was the same as was reported in report ref CE/RCP/PJEH/18094 with no further additions. The results of fatigue tests on 4th stage blades show them to be weak in 2nd flap vibration which leads to the conclusion that the most probable cause of failure is 2nd flap vibration excited by 8 intermediate vanes at 5730 rpm, for the 4th stage blades. Because of exceptionally high resonance on the 4th stage blades it is quite reasonable to believe that the 5th stage blades failed off resonance, thus the blade is very critical
