Description (What the record is about)
-
The case notes appear to have been selected and retained by each doctor for their own information, probably for research. They are organised annually by the year of a patient's discharge or death, although it is worth noting that admission dates can often be much earlier. They are organised within each volume according to a reference number which does not correlate with the sequential number given to patients in the registers. However, the registers do often contain a note of the year and new reference number assigned to those case notes chosen for binding.
The case notes contain the following information in brief: name, age, sex, ward, occupation, residence, private doctor, diagnosis, dates of admission and discharge/death and result (e.g. if recovered). Where relevant, and in more detail, they also include details of previous health, family history, history of present illness, physical signs, treatment/operation, progress and after-treatment and condition on discharge. Additionally, laboratory reports, pathology results and various other medical test results and patient charts are often inserted. Very occasionally, photographs and diagrams showing the patient's problem area/s or symptoms of illness are also included.
Unfortunately, these records bore the brunt of the damage caused by the fire. Consequently, there are a very high proportion of volumes which have been deemed unfit for production.
