Nature of request
[The petition is badly damaged and is missing most of the left-hand side of the petition].The warden requests letters of [...] quit of fine concerning the 60s rent granted to him which issues from land in Blackbrook, Fairfield and Bowden, and which Forde now holds, for the sustenance of the chaplain singing in the chapel of the hospital.

[Philippa of Hainault], Queen [of England]
[Robert de Haustede]

Nicholas de la Forde.
Note
The petition is dated to c. 1343 as it seems likely that it dates closely to the inspeximus and confirmation of a grant in free alms made by Queen Philippa to the petition which dates to 6 July 1343, the original grant being dated to 1 January 1343 (CPR 1343-45, p. 58). Because the petition is badly damaged, it is possible that the petitioner was requesting the confirmation, and that the patent roll entry is the response to it.
