Folios 732-734. From Southwell Union, to the Poor Law Board. Appointment form for a medical officer to the union workhouse completed in respect of John Henry Osborne of Southwell, qualified for 26 years and previously the medical officer to the Basford Union, Calverton district. To be paid 1/6 for each vaccination, on a salary of £35 exclusive of midwifery fees. Form includes details of his medical qualifications (College of Surgeons of England and Society of Apothecaries, London) and names of other medical men residing near the workhouse: Mr Elliott, Mr Cooke and Mr Calvert, all fully qualified. Annotated, note stating that Mr Osborne resigned from the Basford Union on leaving the neighbourhood but salaries and district are the same. Two other notes stating that 'these appointments' should be sanctioned. Also form for the appointment of John Henry Osborne, as Medical Officer, qualified for 26 years, previously employed as in the same post at Calverton district of Basford union, at a salary of £65 plus 10/6 for midwifery cases.
