- Leveson requests an annuity of £10 as he has received no reward for his service with the king in Scotland and Wales and was maimed at the battle of Shrewsbury.

- 17 December 1406 the king with the assent of his council and of other lords spiritual and temporal with the commons assembled in parliament granted that Leveson should have £10 for the term of his life from the issues and revenues of Staffordshire by the hand of the sheriff.
