- The petitioner seeks licence to appropriate lands, tenements and churches worth £40, as well as his own fee as of others in Wales, except lands and tenements held in chief of the king and churches in the king's gift, in order to found a hospital of chaplains to pray for the king and for the souls of all the ancestors and benefactors of the hospital.

- Let him declare in what place he would found this hospital, and of what tenements and advowsons to what value. Let the justice be ordered to enquire if it will be to the king's loss or to that of others if the king makes such a grant. Return this matter and the enquiry to the king, who will grant a day.
