Medical and surgical journal of HM convict ship Forth from 27 April to 23 October 1830 by Joseph Cook, surgeon, during which time the said ship was employed in a voyage from Ireland to Port Jackson, New South Wales.
Folios 1-3: [Jane?] Taylor, aged 32, convict, case number 1; disease or hurt, accidentally fell down hatchway, a depth of 11 feet, receiving an extensive wound penetrating the periasternum about the middle of the left leg. Put on sick list, 29 May 1830 at Cove of Cork. Discharged 22 June 1830 cured.
Folios 4-7: M.A, Rogers, aged 18, convict, case number 2; disease or hurt, frequent shivering headache and sickness having previously suffered with typhus fever. Put on sick list, 2 June 1830 at Cove of Cork. Discharged 18 June 1830 cured.
Folios 7-8: Mary Williams, aged 31, convict, case number 3; disease or hurt, syphillus (suffered with venereal disease last three years). Put on sick list, 5 June 1830 at sea. Discharged 19 June 1830 convalescent.
Folios 9-15: Ann Helpin, aged 19, convict, case number 4; disease or hurt, tetanus (fell on pair of rusty scissors). Put on sick list, 4 July 1830 at sea. Discharged 6 August 1830 cured.
Folios 16-21: Mary Watson, aged 35, convict, case number 5; disease or hurt, hysteria (syncope). This patient was in a delicate state of health when embarked in consequence of confinement in jail and keeping a strong child at the breast for three years. Put on sick list, 11 July 1830 at sea. Discharged 23 October 1830 convalescent.
Folios 22-24: Margaret Buckley, aged 35, convict, case number 6; disease or hurt, dysenteria. Put on sick list, 28 July 1830 at sea. Discharged 6 August 1830 cured.
Folios 25-28: Ann Byrne, aged 35, convict, case number 7; disease or hurt, hysteria. Put on sick list, 3 August 1830 at sea. Discharged 26 August 1830 cured.
Folios 29-31: M.A, Rogers, aged 18, convict, case number 8; disease or hurt, pneumonia. Put on sick list, 27 August 1830 at sea. Discharged 9 September 1830 cured. Folio 31: A numerical abstract of the medical cases mentioned in the journal.
Folios 32-33: Surgeon’s general remarks. In the last week of May 1830 the following persons were embarked on board the Forth at Cove from the penitentiary in Cork, for passage to New South Wales; 120 female convicts, 10 children (belonging to convicts), 8 free women (and 19 children belonging to the free women).
