Produced at the Royal Air Force Station, Kelstern, Lincolnshire, 1 July 1944. Pencilled note: 'Appendix 14 To Form 540. R.A.F, Kelstern'. On dorse: 1 complete copy and 2 partial copies of a printed, coloured, map of the area between Carlisle (Cumberland; now in Cumbria), Kiel (Germany), Augsburg (Germany) and Saint Malo (France).
