Folios 96-97: William Elijah Cook (Navy agent), 13 hart Street, Bloomsbury. Captain Frederick Warren was superceded from the command of HMS Fairy in the West Indies on 26 July and appointed to HMS Amphitrite. He followed her home and joined her on 27 September 1801. Requests payment for him for HMS Fairy for the period 26 July to 27 September.
