Folios 59-60: James Walker, HMS Tartar, South Yarmouth. Lieutenant James Wallace is so ill he needs to go to Sick Quarters. There was not enough time for him to be surveyed at Spithead. Requests that Lieutenant Francis Roskrudge of HMS Prince is appointed in his place.
Folio 61: enclosure with folios 59-60. Direction dated 5 October 1801 from James Walker, HMS Tartar, Senior Officer of ships in Yarmouth Roads, to Surgeons of HMS Tartar, HMS Flora and HMS Amphitrite, to examine Lieutenant Wallace who is said to be sick enough to need to go to hospital.
Folio 62: enclosure with folio 61. Report dated 5 October 1801 to James Walker, HMS Tartar, from William Carpenter, HMS Flora, B. Harris, HMS Amphitrite and Edward Bromley, HMS Tartar. They have surveyed Lieutenant James Wallace and find him incapacitated by general debility and biliousness and believe he should be sent to hospital to recover.
