Folios 205-206: Thomas Wolley, HMS Active, Spithead. Requests a Lieutenant is appointed to replace Charles Forth Wintour, Lieutenant who has been sent to sick quarters on shore with a serious illness, and is unlikely to return before the ship sails on 15 April 1795.
Folio 207-208: enclosure with folios 205-206. Letter dated 10 April 1795 by Thomas Wollet, HMS Active, to the Admiralty. Gives a testimony for Lieutenant Charles Forth Wintour and the regret he feels in having to send him to sick quarters. Commends his aptitude and gallantry in action that would make him a very desirable officer in a ship of a larger class. Believes that Wintour's exertions since the First Lieutenant left has been the principal cause of his illness. Acknowledges the appointment of Lieutenant [George Augustus] Delanoe.
