Folios 257-258: George Tripp, HMS Severn, Spithead. Having visited Mr John Butt, 2nd Lieutenant on HMS Severn, in Hospital, following his accident when he was Officer of the Watch. When the Main Staysail Sheet Block caused a fracture and contusion on our passage from Halifax. Request a replacement Lieutenant be appointed.
