Folios 240-241: Charles Shaw, HMS Philomel, Mediterranean. Enclosing the acting Warrant for David Richards, HMS Philomel and recommending him having lately been severely wounded in gallantly boarding a vessel under the enemies fire.
Folio 242: enclosure with folios 240-241. Warrant for David Richards, Boatswain's Mate on HMS Caledonia, to replace Mr William Cuming, Boatswain HMS Philomel who is Invalided. By Vice Admiral Sir Edward Pellow, HMS Caledonia, Off Toulon. dated 8 October 1812.
