Folio 236: William Stephens, HMS Imogen, Malta. Memorial outlining his action in engagements with the enemy in March and July 1795 as well as serving at the sieges of St Fiorenzo, Bastia and St Maura. Also mentions during the late and current war, the capture and destruction of three Sail of the Line, two frigates, three corvettes and two privateers, and taking and destroying upwards of sixty sail of Merchant vessels as well as engaging with the enemy upwards of 12 times. During this service has sustained injuries, which he ask to be considered, to his foot affecting his walking and was wounded in the head wile quelling a mutiny on HMS Trident in India.

Statement of the case of William Stephens, Captain, HMS Imogen, who was wounded in storming a battery with the troops commanded by General John Oswald on the island of Saint Maura on 22 March 1810. Also a survey of William Stephens's wounds and injuries, conducted at the Naval Hospital at Malta, 26 February 1811 and signed by John Allen, Surgeon, Naval Hospital; Charles Leslie, Surgeon, HMS Aigle; David Moore, Surgeon, HMS Imogen and Thomas Prower, Surgeon, HMS Alceste.
Folio 239: enclosure with folio 236. Letter dated 15 January 1811 by David Moore, Surgeon, HMS Imogen off Corfu. Certifying injuries that William Stephens, Captain, HMS Imogen, sustained storming a battery under the command of General John Oswald on the island of Saint Maura.
