Accidents: inspecting officers' reports and Parliamentary returns, includes report on head on train crash at Forden in 1904 with plan of station and track layout, and also with crash detail (date 27 Nov 1904). (Described at item level).

Note (Additional information about the record)
- In October 2014 it was noticed that item numbers on the online Catalogue for this piece had been incorrectly loaded with regard to items 5-11 which had resulted in them being out of sequence. This has now been corrected for those items in the piece (so that items 5-11 are now items 33-39).

Physically each piece comprises 4 quarterly sets of reports, each of which is divided into Appendices A and B (and sometimes C). For the purpose of cataloguing, all those accident reports in A appendices have been grouped together and appear first in the listing. These are followed by reports from all four B and C appendices. The quarter, appendix and page number for each accident is clearly given in the item description. The narrative descriptions for the major accidents in Appendix A provide full details including the circumstances of the accident, whereas that detail is omitted for the more minor accidents in Appendices B and C, the descriptions for which are in the form of a more structured data format.
