Folios 274-275: Paul Minchin, HMS Severn. Reports that Mr James Jack, First Lieutenant of HMS Severn, became insane soon after they sailed from Quebec, and remained that way for 3 weeks. He still suffers from episodes, and the Second Lieutenant has been performing his duties. Requests appointment of a repalcement. Mr Henry Carew, Second Lieutenant, would be his first choice.
