Locomotive Department: Record of Accidents Volume No. 5, listing accidents to staff including drivers, firemen, cleaners, apprentices, labourers, boilermakers, fitters, etc., giving name, date of accident, occupation and foreman's report (which includes the age, rate of pay and address of the accident victim), place of accident, result of accident and, where applicable, details of inquest. (Described at item level).
