Occupation: Mineral Brakesman.
Place of accident: Llantrisant (Taff Vale) Goods Yard, Glamorgan.
Injury: Broken arm, fractured thigh and internal injuries.
Details of accident: As a wagon was proceeding to a siding with wagons to attach to its train, Lewis got on to the engine step and was crushed between the engine and the end wagon on the main line which was standing near the crossing.
