Name: Cuthbert Stephens.
Occupation: Lamp lad.
Place of accident: Llanishen, Glamorgan.
Injury: Lip cut.
Details of accident: Stephens, while riding on a light engine in defiance of instructions, accidentally touched the reversing lever spring, causing the lever to fly out and strike him on the lip, causing the injuries described. Stephens resumed work on the 4th September.
