Name: Ivor Llewellyn.
Occupation: Locomotive Engine Cleaner.
Place of accident: Senghenydd, Glamorgan.
Injury: Left forearm squeezed.
Details of accident: Llawellyn, when coupling two Engines, one of which was in steam and being moved by the assistant Foreman, negligently placed his arm over the cap of the buffer with a result that his left forearm, just above the wrist was caught between the buffers.
