Place of accident: Bargoed Pits, Glamorgan.
Injury: Fatal.
Details of accident: Samuel was engaged forming his train on a relief road. After signalling to the driver to push up with five wagons to couple on to a tank wagon, he attempted to cross the metals between the tank and the remainder of his train and was caught between the buffers.
