Condition/Injury/Disease: Shell Shock; Appendicitis.

Details and Outcome: Patient admitted suffering with shell shock. Arrived at Catterick Command Depot on 11 June 1918. Admitted to Catterick Military Hospital suffering with suspected appendicitis on 18 June 1918. Further details given. Discharged to Draft on 3 September 1918.
