Service number: 84285.

Hospital(s): 4th Northern Genera Hospital, Lincoln; Royal Artillery and Tank Corps Command Depot, Catterick; Catterick Military Hospital.

Condition/Injury/Disease: Gas Poisoning.

Details and Outcome: Patient admitted suffering with gas poisoning. Arrived at Catterick Command Depot on 25 January 1918. Further details including ophthalmic report given. Discharged to Draft on 1 May 1918. Civil occupation - Gardener. Includes a medical case sheet only.
