Hospital(s): Colchester Military Hospital; Great Eastern Military Hospital, Harwich; Horton War Hospital, Epsom, Surrey; Royal Artillery and Tank Corps Command Depot, Catterick (Royal Artillery Collecting Station, Hipswell Camp)

Condition/Injury/Disease: Gunshot Wound to Eye.

Details and Outcome: Patient admitted suffering with a gunshot wound to the left eye. Under course of treatment patient was fitted wit an artificial eye. Arrived at Catterick Command Depot 3 January 1918. Further details given. Discharged to Draft 24 April 1918. Civil Occupation: Miner. Includes a medical case sheet only.
