Service number: 38760 [or 38768].

Age: 29.

Hospital(s): 3rd London General Hospital, Wandsworth; Brooklands Military Hospital, Weybridge.

Condition/Injury/Disease: Hammer Toes; Bronchial Catarrh (Broncho-Pneumonia).

Details and Outcome: patient admitted for two courses of treatment suffering from hammer toes and bronchial catarrh. During first course of treatment patient underwent amputation of right second toe and remove the proximal phalanx of the left second toe. During second course of treatment patient underwent operation to remove both nasal turbinates. Further details given. Patient discharge [on home furlough] 5 August 1916. Includes two medical case sheets.
