Unit/Battalion/Regiment: 69 Divisional Ammunition Column, Royal Field Artillery.

Service number: 38745.

Age: 35.

Hospital(s): Wharncliffe War Hospital, Sheffield; 3rd Northern General Hospital, Sheffield.

Condition/Injury/Disease: Fractures of Forearm.

Details and Outcome: Patient admitted for two courses of treatment suffering from fractures to both bones of the left forearm sustained while shoeing a horse at La Bourse. During course of treatment patient underwent multiple X-rays and operations. Further details given. Contains sketch of fractured bones. Discharged as permanently unfit 2 November 1917. Includes two medical case sheets, two clinical charts and four X-ray reports.
