Name: Thomas Lewis.
Occupation: Goods Guard.
Place of accident: Brecon, Brecknockshire.
Injury: Loss of feet.
Details of accident: Lewis attempted to put down the brake of one of the wagons of a Midland Goods train which was being shunted in the Brecon yard, when he slipped and fell under the train. One of his feet was cut off and the other so injured that it had to be amputated.
