Details of accident: When loading a case of Machinery ( about six tons) in Pwllheli Warehouse the chain hooks by some means slipped as the case was being lowered by the crane on the lorry, and the case fell partly on Allen, causing a few slight bruised and cuts and a small bone fracture in his ankle.
