Name: John J Tulip.
Occupation: Boilermaker.
Place of accident: Barry, Fitting Shop, Glamorgan.
Injury: Slight cut on ball of left eye.
Details of accident: A lever was being taken off locomotive ashpan door and Tulip was holding the door for another boiler-maker to punch out split pin. The punch rebounded and struck Tulip in his eye.
