Name: Frederick Pitman.
Occupation: Washer out.
Place of accident: Barry, Trehafod Engine Shed, Glamorgan.
Injury: Slight bruise on back of head.
Details of accident: When preparing No.30 Locomotive for washing out he was slackening the right hand bottom front washout plug with a spanner, when the latter slipped off the plug, and he stumbled backwards, the back of his head striking eccentric strap.
