Name: Alfred William Lewis.

Rank: Lance Corporal.

Unit/Battalion/Regiment: 17 Lancers, B Squadron.

Service number: 3527.

Age: 33.

Hospital(s): 2 Southern General Hospital, Bristol; Gourney Court V A Hospital, West Harptree.

Condition/Injury/Disease: Malaria.

Details and Outcome: Includes second medical case sheet, two clinical charts and a note of admission and discharge from Gourney Court Hospital. Patient went from South Africa to India in 1902, then to France, end October 1915. Has had attacks of malaria, the last one coming over on the boat. Transferred to Gourney Court Hospital, 08/03/1915. Treatment prescribed, details given. Improved by 03/09/1915. Discharged to duty.
