Name: R F MacKenzie.
Occupation: Locomotive Fireman.
Place of accident: No 6 Empty Road, Barry Dock No 2, Glamorgan.
Injury: Two fingers severed after being trapped under the wheels of an engine.
Details of accident: Whilst climbing down from the engine to clear a blockage in the sand pipe, MacKenzie slipped and fell with his right hand passing under the wheels and severing two fingers.
