Apart from most of the transcripts and the published reports, the majority of the records contain personally sensitive medical details which are closed for the anticipated lifetime of the individuals concerned. Records relating to the administrative support to the inquiry were not selected.

Administrative / biographical background

Appointed by the Secretary of State for Health in Feb 1997 to investigate the functioning of the Personality Disorder Unit (PDU) at Ashworth Special Hospital, the inquiry was chaired by His Honour Peter Fallon QC, former Senior Circuit Judge and Recorder of Bristol. It investigated allegations made by a former patient, Steven Daggett, about the misuse of drugs and alcohol, financial irregularities, possible paedophile activity and the availability of pornographic material on the Unit. In the light of their investigations, the inquiry was also asked to review the following: policies, clinical care and procedures on the Unit; security arrangements; management arrangements for assuring effective clinical care and appropriate security for patients; and arrangements for visiting on the Unit. The report of the inquiry was presented to Parliament (Cm 4194-II), in January 1999.
