He encloses for approval the following appointment forms.Appointment form for William Heywood Paulson as medical officer for the Number 2 District. His place of residence is Mansfield. Member of the London College of Surgeons and Licentiate of Apothecaries Company.
Appointment form for Henry Isaac Lomax as medical officer for the Number 4 District. His place of residence is Sutton-in-Ashfield. Member of the London College of Surgeons and Licentiate of the Society of Apothecaries.
Appointment form for Griffin Spencer as medical officer for the Number 5 District. His place of residence is Alfreton, [Derbyshire]. Has a warrant or commission as a surgeon, assistant surgeon or apothecary in Her Majesty's Navy or Army or the Honourable East India Company.
