Folios 378-379: John Perkins, 22 Arundel Street, Strand, [London]. He has served 47 years in the West Indies, 45 of those on the Jamaica station. His good character and conduct can be confirmed by the different Admirals and Generals who have commanded in the West Indies. For some years past he has been troubled by asthma which was why he served in a warm climate. When HMS Tartar which he had commanded for 3 years was ordered to the Halifax station in December 1804 the Surgeon gave a certificate that a cold climate would be the death of him and he was obliged to give up the ship. He had leave from Lord Barham to remain in Jamaica for the benefit of his health. Offers to serve in the West Indies. Since his arrival he has had 2 or 3 attacks of asthma and the Surgeons think he will suffer in the winter. Asks to be allowed to return to the West Indies and asks for a letter to Vice Admiral Dacres to put him on the list for an Admiralty vacancy.
