Folios 344-345: Edward William Campbell Rich Owen, London. Captain Benjamin Warburton, formerly a Lieutenant of HMS Clyde, has submitted a memorial concerning an injury sustained while serving on HMS Clyde and seeking a pension for that service, also encloses a report from Sir Thomas Bellinghurst, Surgeon on HMS Clyde, giving details of the fall sustained and which were reported at the time on 22nd October 1807.
Folio 346: enclosure with folios 344-345. Certificate dated 1801 from Sir Thomas Bellinghurst, Surgeon, of HMS Clyde that Lieutenant Benjamin Warburton was injured on the night of 22nd October 1807 falling from the Quarter deck to the Main deck inducing partial paralysis.
