Folios 63-64: Charles Herbert, HMS Uranie, Plymouth. On the advice of the Surgeon of HMS Vigilant he has taken Captain Crofts on board as he is suffering from a West Indies fever which is likely to prove fatal if he commands the ship on her voyage to Jamaica. A survey conducted by the Surgeons of HMS Uranie, Elephant and Inconstant has confirmed this assessment. He has ordered the 1st Lieutenant of HMS Vigilant to take command of the ship until she reaches Jamaica.
Folios 65-66: enclosed with folios 63-64. A medical survey report dated 20 August 1804 on HMS Vigilante with regards to Captain William Croft, HMS Vigilant conducted by the Surgeons of HMS Uranie, HMS Elephant and HMS Inconstant.
