Folios 75-76: John Dick, HMS Discovery, Nore. Mr Phillips, Boatswain of HM Bomb Vessel HMS Discovery being in a very bad state of health and having received a letter written by desire of Lord Spencer to state his case officially. Encloses his letter to him and states his total incapability of doing his duty having quitted HMS Orestes.
Folios 77-78: enclosures with folios 75-76. Letter dated 23 May 1799 by Thomas Phillips, HMS Discovery, Nore, to Captain John Dick, HMS Discovery. Having received a letter from the Admiralty dated 20 May 1799 where he is requested to send the state of his health officially. Therefore requests you give me a certificate for that purpose as his health will not permit him to remain in active service.
