Folio 194: Thomas Briggs, HMS Orpheus, Madeira. Encloses a survey on the health of Lieutenant William Jones of HMS Venus together with Captain Henry Matson's letter and another from Surgeon Robert Mulberry.
Folios 195-197: enclosed with folio 194. Letter dated 19 August 1806 by Robert Mulberry, Surgeon, HMS Venus, to Captain Henry Matson, Madeira. Lieutenant William Jones is suffering from rheumatism and piles and his worsening health makes him incapable of performing his duty. He was [removed ?] from HMS [illegible] for the same complaint on September 15, 1802. Asks for his health to be surveyed.

Folio 197: enclosed with folio 194. Letter dated 19 August 1806 by Captain Henry Matson, HMS Venus, Funchal Bay, Madeira, to Captain Thomas Briggs of HMS Orpheus, Senior Officer at Madeira. Surgeon Robert Mulberry reports that Lieutenant William Jones is suffering from rheumatism, piles and partial blindness and asks that his health be surveyed.
Folios 198-199: enclosed with folio 194. Letter dated 19 August 1806 by Captain Thomas Briggs, HMS Orpheus, Madeira, to Commander Francis Collier and Surgeon Joseph [Ollett ?] of HMS Wolverine and Surgeon William Maybank of HMS Orpheus. Captain Henry Matson of HMS Venus has reported that the health of Lieutenant William Jones is much impaired, orders them to survey his health and report whether he is fit for duty. If he is not fit, they should report their reasons and be prepared to swear to them. He encloses Captain Matson's letter and that from the Surgeon of HMS Venus.
Folio 199: enclosed with folio 194. Letter dated 19 August 1806 by Commander Francis Collier and Surgeon Joseph [Ollett ?] of HMS Wolverine and Surgeon William Maybank of HMS Orpheus, HMS Venus, Madeira. As directed by Captain Thomas Briggs, they have been aboard HMS Venus to survey the health of Lieutenant William Jones, and find that he is suffering from rheumatism, piles and dimness of sight in both eyes and is therefore incapable of performing his duty. They are prepared to swear to these findings on oath.
