Folios 261-264: William Butterfield, HMS Hazard, Plymouth. States that he is satisfied that Andrew Elphinstone, late Surgeon of HMS Hazard, did everything his power to join as soon as he has been informed by Admiral Kingsmill [Robert Brice Kingsmill] where she was.
Folio 262: enclosure with folios 261-262. Letter dated 14 December 1798 from Andrew Elphinstone, Surgeon, Plymouth Dock to Evan Nepean, the Admiralty. States that having been left behind by accident by HMS Hazard at Cork Harbour, he took every possible dispatch to join her and, as soon as he was informed by Vice Admiral Robert Brice Kingsmill where she was, he arrived at Plymouth on 7 December 1798 to find that he had been superseded and an 'R' placed against his name. Says he did not know the official procedure at the time but did write immediately to the Board of Sick and Hurt stating what had happened and the steps he had taken to rejoin the ship. States he has served 2 years with HMS Hazard and asks for the'R' to be removed from the ship's books.
