Folio 1: Printed instructions on completing nosological tables, recording where a patient was taken ill, dividing the General Remarks into three sections relating to the ship and crew, her movements and places visited and a consideration of the diseases.
Folio 2: case no 1, no on ship's book 5-97, George Lawrence, aged 29, Caulker's Mate; taken ill at Singapore; sick or hurt, debility after typhoid fever [this case was carried on from last journal]; sent 7 January 1875 to Colonial Hospital at Singapore.
Folios 2-3: case no 2, no on ship's book 15-54, Henry Richards, aged 18, Ordinary Seaman; taken ill at Singapore; sick or hurt, eczema of right ankle and foot with numerous sore and infiltration of the integumentes; sent 7 January 1875 to Colonial Hospital at Singapore.
Folios 3-4: case no 3, no on ship's book 5-42, Denis McCarthy (no 2), aged 23, Able Seaman; taken ill at Singapore; sick or hurt, primary syphilis, open bubo and gonorrhoea; put on sick list 12 December 1874, sent 7 January 1875 to Colonial Hospital at Singapore.

Folios 4-6: case no 5, no on ship's book 5-150, John Merrett, aged 30, Stoker; taken ill at [Singapore]; sick or hurt, fractured of base & vault of skull across 10th dorsal vertibra & neck of 11th rib, he was unscrewing the nut confined around the funnel of the engine, the last threads of the screw being worn the band unclasped suddenly and he fell backwards struck against the ash shute; put on sick list 15 January 1875, died 16 January 1875 at 3.30 am [with post mortem reported].
