Folio 1: Printed instructions on completing nosological tables, recording where a patient was taken ill, dividing the General Remarks into three sections relating to the ship and crew, her movements and places visited and a consideration of the diseases.

Folio 2: case no 1, no on ship's book 5-97, George Lawrence, aged 29, Caulker's Mate; taken ill at Singapore; sick or hurt, debility after typhoid fever [this case was carried on from last journal]; sent 7 January 1875 to Colonial Hospital at Singapore.

Folios 2-3: case no 2, no on ship's book 15-54, Henry Richards, aged 18, Ordinary Seaman; taken ill at Singapore; sick or hurt, eczema of right ankle and foot with numerous sore and infiltration of the integumentes; sent 7 January 1875 to Colonial Hospital at Singapore.

Folios 3-4: case no 3, no on ship's book 5-42, Denis McCarthy (no 2), aged 23, Able Seaman; taken ill at Singapore; sick or hurt, primary syphilis, open bubo and gonorrhoea; put on sick list 12 December 1874, sent 7 January 1875 to Colonial Hospital at Singapore.

Folios 4-6: case no 5, no on ship's book 5-150, John Merrett, aged 30, Stoker; taken ill at [Singapore]; sick or hurt, fractured of base & vault of skull across 10th dorsal vertibra & neck of 11th rib, he was unscrewing the nut confined around the funnel of the engine, the last threads of the screw being worn the band unclasped suddenly and he fell backwards struck against the ash shute; put on sick list 15 January 1875, died 16 January 1875 at 3.30 am [with post mortem reported].

Folio 10: case no 13, no on ship's book 5-145, Donald Marray, aged 31, Captain of Hold; taken ill at Singapore; sick or hurt, chronic diarrhoea; put on sick list 2 March 1875, sent 21 April 1875 to Royal Naval Hospital st Hong Kong, returned 8 May 1875.

Folios 10-11: case no 14, no on ship's book 5-141, Robert Cook, aged 35, Able Seaman; taken ill at [Singapore?]; sick or hurt, chronic headache with pain in the vertex, tremor of hands and tongue, nervouse depression, thickness of speech; put on sick list 1 April 1875, sent 21 April 1875 to Royal Naval Hospital at Hong Kong, invalided 3 May 1875 at Haslar Hospital.

Folio 11: case no 15, no on ship's book 5-69, Patrick Lee, aged 18, Ordinary Seaman; taken ill at sea; sick or hurt, sympathetic phlegmon; put on sick list 7 April 1875, sent 26 April 1875 to Hospital, returned 8 May 1875 and joined this ship to Japan.

Folio 11: case no 16, no on ship's book 3-2, Mr Charles Stratford, aged 45, Chief Engineer; taken ill at Hong Kong; sick or hurt, palpitation and melancholia; put on sick list 23 April 1875, sent 26 April 1875 to Hospital, submitted for invaliding 9 July 1875 for debility, before Fleet Surgeon Davis, the Surgeon [Thomas Leaman Bickford] and Surgeon Hamlye of Frolic.
