Folios 172-179. Letter from Stephen A Cooke, Chardstock Vicarage, Near Chard, Somerset, Minister for the parish of Chardstock to the Poor Law Commission enclosing two witness statements regarding William Walden, deceased, aged 34 of the parish of Chardstock and the alleged neglect of Mr Hayman, of Axminster, Medical Officer for the parish of Chardstock. Cooke states circumstances leading up to Walden's death and churchwardens and parish officers concur with Cooke in considering this a case of 'gross neglect'. Witness statement from Eliza Perham who attended the Walden family. Perham gives details of dates of visits during March and April 1846 of Mr Jolliffe, Mr Hayman and Mr Wills [Hayman's apprentice] all Medical Officers. Perham details that on 8 April 1846 she collected two powders for Walden from the home of Samuel Pinney, another sick man in the parish of Chardstock. Witness statement of Robert Pavey who attended Walden from 10 April 1846 and describes situation in which he found Walden. On 12 April 1846 Pavey saw the overseer, Mr Bonfield at the George Inn, Chardstock. Also present were Mr Thomas Deane; Mr Joseph Deane; Mr John Deane, Churchwarden; Mr Noah Bishop, Guardian and Mr John Marks. Bonfield ordered Pavey to go for Mr Jolliffe, late Medical Officer for the parish residing in Chard. Jolliffe visited Walden the same evening at his home at Berridge [Burridge], in the parish of Chardstock. Jolliffe instructed Pavey to remain with Walden as he did not expect him to live through the night. Pavey stayed with Walden until he died the next evening [13 April]. Pavey states the Wills visited on 13 April 1846 in the middle of the day to see Walden's sister and describes the conversation between Pavey and Wills. Pavey states William Walden died at 7pm on 13 April 1846 and Jolliffe visited as the corpse was being laid out.
