Folio 4: case no 23, Charles Sherwin, aged 18, Boy 1st Class; taken ill at Ningpo; sick or hurt, remittent fever; put on sick list, 11 November 1857, sent 28 November 1857 to Hospital Ship Hercules at Hong Kong.

Folio 4A: case no 24, James Crow, aged 26, Chinese Stoker; taken ill at Hong Kong; sick or hurt, syphilis; put on sick list, 1 December 1857, sent 3 December 1857 to Hospital Ship Hercules at Hong Kong.

Folio 4A: case no 25, Eneder Hawke, aged 40, Caulker; taken ill off Tiger Island; sick or hurt, intermittent fever; put on sick list, 5 December 1857, sent 7 December 1857 to Temporary Hospital at Wang Tung Island, Canton River.

Folios 10-12: Surgeon's general remarks, on topographical of Shanghai and surrounding regions, he mentioned that the Chinese were ignorant of the treatment of the venereal disease, also the small pox was prevalent but a respectable native physician from Foochow was taught the mode of vaccination at the Chinese Hospital at Shanghai with supplied of lymph, lancet and the pamphlet on vaccination written by Dr Pearson and Sir George Staunton. He soon afterwards vaccinated 800 children at Foochow. The Surgeon went on describing the filthy state of street and canals in the city of Shanghai. Included in the general remarks is the Surgeon's suggestion to supply filters for the lower deck.
