Folios 11-12: Case no. 1, Benjamin Bennett, aged 45, Able Seaman; taken ill at Baljick; disease or hurt, carditis chron:, pain in the superior anterior part of right side of chest with troublesome dry cough, Put on sick list, 20 July 1854, Died 10 August 1854 at 5 pm.

Folios 12-13: Case no. 2, George Boucher, aged 36, Marine; taken ill at Baljick; disease or hurt, cholera, griping pin in belly with frequent purging, denied having eaten any thing except his usual food, had no nausea, Put on sick list, 7 August 1854, Died 7 August 1854 at 5.15 pm.

Folios 13-14: Case no. 3, Nathaniel Higgins, aged 22, Marine; taken ill at sea; disease or hurt, cholera, cramps across the epigastrium and in the limbs, Put on sick list, 11 August 1854, Died 11 August 1854 at 11.45 pm.

Folios 17-18: Case no. 6, Mr. William Sparks, aged 46, Gunner; taken ill at sea; disease or hurt, cholera, Put on sick list, 14 August 1854, discharged 3 September 1854 to duty.

Folios 18-19: Case no. 7, Percy Smythe, aged 40, Boatswain’s Mate; taken ill at sea; disease or hurt, cholera, Put on sick list, 15 August 1854, died 16 August 1854 at 9.15 am. [within folio 19, there is a piece of paper written on it “no. victualled 43,813. 53 salt meat days…].

Folio 25: Case no. 13, Jesse Ward, aged 25, Able Seaman; taken ill off Crimea; disease or hurt, cholera, was employed after the battle of Alma in carrying in the wounded and had suffered ever since from looseness of bowels; had no griping or pain in the belly, Put on sick list, 23 September 1854, discharged 8 October 1854 to duty.

Folios 26-27: Case no. 14, William Gooch, aged 19, Marine; taken ill off Crimea; disease or hurt, cholera, was employed on shore at the Alma on 21 September, said he had caught cold and looseness in the bowels and now attacked with severe griping pains in stomach and bowels, Put on sick list, 24 September 1854, discharged 21 December 1854 to duty.

Folios 30-31: Case no. 17, Joseph Andrews, aged 19, Ordinary Seaman; taken ill off Crimea; disease or hurt, cholera, Put on sick list, 1 October 1854, discharged 8 October 1854 to duty.

Folio 31: Case no. 18, Samuel Johnson, aged 33, Able Seaman; taken ill off Katchkar River; disease or hurt, wound of the scalp which caused severe headache and deafness, Put on sick list, 26 September 1854, sent 18 October 1854 to Hospital at Therapia.

Folio 31: Case no. 19, Robert Bakers, aged 17, Boy 1st Class; taken ill off Sebastopol; disease or hurt, erysipelas, the inflammation extended from foot along leg and thigh, Put on sick list, 8 October 1854, sent 18 October 1854 to Hospital at Therapia.

Folio 31: Case no. 20, John Warrington, aged 35, Private Royal Marine; taken ill off Sebastopol; disease or hurt, rheumatism and ulcer, Put on sick list, 8 October 1854, invalided 11 November 1854.

Folio 31: Case no. 21, Henry Brandon, aged 20, Ordinary Seaman; taken ill off Katchkar River; disease or hurt, wound on the leg from which troublesome ulcer resulted, Put on sick list, 9 October 1854, sent 5 December 1854 to Hospital at Therapia.

Folio 31: Case no. 22, James Penicud, aged 36, Ship’s Steward; taken ill off Sebastopol; disease or hurt, rheumatism, Put on sick list, 10 October 1854, sent 18 October 1854 to Hospital at Therapia.

Folio 31: Case no. 23, Thomas Cox, aged 25, Butcher; taken ill off Sebastopol; disease or hurt, pompholyx, an eruption over arms & legs just after he was kill some cattle, on 17 October, this patient with other sick was sent on board HMS Vulcan (prior to the fleets weighing to attack the fort, and on following day it was though advisable to send him on in her to Therapia Hospital, Put on sick list, 14 October 1854, sent 18 October 1854 to Hospital at Therapia.

Folios 32-33: The following 12 cases [case nos 24-35] are casualties which occurred on October 17, during bombardment of Forts of Sebastopol by combined Fleets.

Folio 32: Case no. 24, Henry Penfold, aged 24, Able Seaman; taken ill off Sebastopol; disease or hurt, killed by gunshot wounds, extensive laceration of both thigh and pelvis, Put on sick list, 17 October 1854, died 17 October 1854.

Folio 32: Case no. 25, Michael Curtain, aged 20, Ordinary Seaman; taken ill off Sebastopol; disease or hurt, gunshot wounds, whole of fleshy part of right hip, ilium, sacrum and lumbar region carried away, Put on sick list, 17 October 1854, died 17 October 1854.

Folio 32: Case no. 26, George Lans, aged 23, Able Seaman; taken ill off Sebastopol; disease or hurt, gunshot wound, struck by a shot over cardiac region causing fracture of several ribs, Put on sick list, 17 October 1854, died 17 October 1854.
