Folio 12: case no 91, Richard Rolleston, aged 20, Able Seaman; sick or hurt, vulnus of sole of foot; put on sick list 8 August 1840, discharged 11 August 1840 to duty.

Folio 12: case no 97, George Fowler, aged 49, Carpenter; sick or hurt, ulcus – varicose of right leg, carrying it for a year and showed it for the first time, much congested & surrounded by inflamed of limb – varicose veins existing 15 years; put on sick list 15 August 1840, discharged 31 August 1840 to duty.

Folios 12-13: case no 98, James Clements, aged 43, Sergeant Marine; taken ill at St John's New Found Land; sick or hurt, vulnus (penetrating abdomen) – while assisting the Master at Arms to put John Collins in irons, he received a stab in the belly by the prisoner with his clasp knife. The Surgeon seeing the wound, which was already examined by the Surgeon of the Snake, said the poole entered two inches and he supposed it to have penetrated the cavity. The wound was ¾ inch long, situated in front of the intercostal space of the 8th and 9th ribs of the left side, crossing obliquely the direction of the rectus, it bled very little, the patient was greatly depressed and anxious and despaired of himself…the patient made water two or three times; had a painless call to stool, he continued easy, conversed rationally – but gradually sunk during the evening; put on sick list 22 August 1840, died 23 August 1840, according to the Surgeon, the murderer was tried by Court Marshall [Court Martial] at Quebec in September, and sentenced to death and executed on the 28 [September], in the presence of the squadron.

Folios 14-15: case no 99, George Goding, aged 27, Quarter Master; taken ill at St John's New Found Land; sick or hurt, ulcus (fungoid of testicle) – with the right testicla enormously enlarged, painful and of strong hardness and uniform in its feel; put on sick list 22 August 1840, sent 28 October 1840 to Bermuda Hospital.

Folio 15: July 1840, sailed on 12 July from Halifax, anchored in St John's Newfoundland on 16 July and sailed on 27 July to Quebec.

Folio 15: case no 104, John Harold, aged 22, Ordinary Seaman; sick or hurt, phlegmon of the gum; put on sick list 1 September 1840, discharged 3 September 1840 to duty.

Folio 15: case no 105, John Lighton, aged 42, Marine; sick or hurt, colica, indigestion and put on sick list 9 September 1840, discharged 15 September 1840 to duty.

Folio 15: case no 106, David McKight, aged 25, Midshipman's Cook; sick or hurt, catarrh with rheumatism; put on sick list 11 September 1840, discharged 15 September 1840 to duty.

Folios 15-16: case no 107, Stephen Lushington, aged 37, Captain; sick or hurt, bubo (with sinuous abscesses), continued from case no 57, he rejoined the ship from Bermuda Hospital on 14 July – much reduced in strength and flesh…from 12 July to 11 August he was on shore in Hospital to resumed treatment, from 11 to 22 August he was on board this ship continued treatment, as the ship about to sail for Quebec he was left ashore in [St Quortors], he waited her return with a gaping wound, the ship returned on 16 October from Quebec, his wound had healed, but the inner and upper portion showed a disposition to become simious…the patient received peremptory order from the Admiralty to proceed to the West Indies to take the active command of the ship or to be invalided, if not able to do so, he was invalided on 8 October 1840 at St John's Newfoundland.

Folios 16-17: case no 108, David Butterfield, aged 21, Ordinary Seaman; taken ill at sea; sick or hurt, bubo (idiopathic), a Bermudian ‘negro', robust and sound constitution, had the glands in the left groin inflamed, large, hot and painful…; put on sick list 12 September 1840, sent 5 November 1840 to Bermuda Hospital, he rejoined the ship on 11 December 1840 at Bermuda, fat and strong.

Folio 17: case no 109, Charles Jennings, aged 24, Able Seaman; taken ill at Quebec; sick or hurt, hemorrhois, abscess on verge of anus; put on sick list 14 September 1840, discharged 24 September 1840 to duty.
