Folios 1-3: case no 1, John Taylor, aged 26, convict, taken ill in River Thames; sick or hurt, bronchitis; put on sick list 30 October 1840, discharged 3 December 1840.
Folio 3: case no 2, Henry Stringer, aged 21, convict, taken ill at Sheerness; sick or hurt, cholera; put on sick list 2 November 1840, discharged 5 November 1840 quite well.
Folios 3-4: case no 3, William Collins, aged 26, soldier, taken ill at Sheerness; sick or hurt, pneumonitis; put on sick list 4 November 1840, died 9 November 1840 at noon, predicted his dissolution from the beginning. There was no post mortem on account of his wife and the inconvenience of the place.
Folios 4-5: case no 4, Patrick Davis, aged 23, soldier, taken ill at the Nore; sick or hurt, pneumonia; put on sick list 7 November 1840, discharged 25 November 1840 to duty.
Folios 5-8: case no 5, John Williams, aged 29, convict, taken ill at Spithead; sick or hurt, opthalmia, lost the sight of his right eye about 2 years ago by inflammation, and the left is now considerably inflamed, taken a purgative and applied a goulard wash; put on sick list 25 November 1840, discharged 24 January 1841, eyelid quite well, inflammation of eye gone, but small nebula still remains on the superior and external part of the cornea.
Folios 8-9: case no 6, Charles Evans, aged 19, convict, taken ill at Spithead; sick or hurt, luxation, by falling down the main hatchway ladder, dislocated the left humerous which he says was twice laxated before; put on sick list 26 November 1840, discharged 3 December 1840 quite well.
Folios 9-10: case no 7, William Provis, aged 29, convict, taken ill at sea; sick or hurt, fever, of a miserable emaciated appearance, complained of headache and general malaise, which he has taken some purgative medicine; put on sick list 13 December 1840, died 30 December 1840 at 6 pm, the body was not examined in consequence of inconvenience of the place and the extreme heat of the weather..
Folios 10-12: case no 8, Thomas Cull, aged 18, convict, taken ill at sea; sick or hurt, phthisis incipiens; put on sick list 7 January 1841, discharged 28 February 1841 well.
Folios 12-18: case no 9, Thomas Raymont, aged 21, convict, taken ill at sea; sick or hurt, obstipatio, costive bowels took some salt which did not operate, yesterday he took some jalop which operated once last night complained of severe pain in the abdomen; put on sick list 9 January 1841, discharged 20 February 1841 quite well.
Folios 18-20: case no 10, John Smith, aged 21, sailor, taken ill at sea; sick or hurt, phthisis, a miserable looking, dirty, emaciated wretch, has been on the list since 14th Inst. With cough brought on by drunkeness and laying about the deck, he has taken pectoral medicine and an opiate; put on sick list 29 January 1841, died 5 March 1841 at noon, no post mortem in consequence of my right hand being disabled by a phlegmon on the index finger.
Folios 20-22: case no 11, John Harris, aged 32, convict, taken ill at sea; sick or hurt, dyspepsia, complained of irregularity of the digestive organs, he took a cathartic a few days ago which operated and yesterday not being better he took an ametic which operated freely; put on sick list 7 February 1841, died 4 March 1841 at noon, not being able to examined the body, from a very painful phlegmon on the index finger of my right hand.
Folios 22-23: case no 12, Walter Williams, aged 34, convict, taken ill in Storm Bay; sick or hurt, pneumonia; put on sick list 16 March 1841, sent 24 March 1841 to hospital.
Folio 23: case no 13, Denis Regan, aged 20, convict, taken ill at Hobart Town; sick or hurt, epilepsy, has been subject to epileptic fits from his youth and had them repeatedly on the passage out, was found dead in his bed this morning laying on his face, he was suffocated in a fit; put on sick list 23 March 1841, died 23 March 1841 the body was taken on shore and an inquest held on it, but no autopsy.
