Folios 7-8: case no 1, John Burnes, aged 20, soldier of the guard, taken ill at sea; sick or hurt, opthalmia, slight inflammation of both eyes more particularly in the right eye; put on sick list 28 August 1842, discharged 6 September 1842 to duty.
Folios 8-9: case no 2, John Cook, aged 47, prisoner, taken ill at sea; sick or hurt, phthisis, had cough and expectoration for the last six months, he was a warn out looking man of a sallow complexion and much emaciated, had now frequent cough, chiefly in the morning with purulent expectoration, decubitus on right side, appetite bad, occasional diarrhoea; put on sick list 2 September 1842, died 3 October 1842 at 7 am.
Folios 9-10: case no 3, Nowal Williams, aged 19, prisoner, taken ill at sea; sick or hurt, cynanche tonsillaris, inflammation of the tonsils, deglutition painful, slight headache and acceleration of pulse; put on sick list 29 August 1842, discharged 12 September 1842 cured.
Folio 10: case no 4, A. Yates, aged 19, prisoner, taken ill at sea; sick or hurt, diarrhoea, complained of purging frequently in the night; put on sick list 6 September 1842, discharged 8 September 1842 cured.
Folio 10: case no 5, Henry Skinner, aged 24, prisoner, taken ill at sea; sick or hurt, ephemera, had smart febrile action, skin cool; put on sick list 20 September 1842, discharged 22 September 1842 cured.
Folios 10-12: case no 6, Thomas Vincent, aged 24, prisoner, taken ill at sea; sick or hurt, pleuritis, pain over a large surface of the right side increased on the slightest motion or a full inspiration, had no cough nor headache; put on sick list 30 September 1842, discharged 21 October 1842 cured.
Folio 12: case no 7, William Wilmot, aged 26, prisoner, taken ill at sea; sick or hurt, hemoptysis, felt tightness and some pain of chest, on exerting himself in lifting a weight coughed up some blood, the haemorrhage continued in moderate quantity with a florid, frothy appearance; put on sick list 9 October 1842, discharged 15 October 1842 cured.
Folios 12-14: case no 8, Charles Dalladay, aged 51, prisoner, taken ill at sea; sick or hurt, scorbutus, an infirmed old looking man, complained of debility and sore mouth, gums swollen, spongy and of florid hue, separated from the teeth and projecting upwards on each tooth like the granulations of an ill conditioned ulcer, mastication impeded and lower jaw more affected than the upper one, pain of limbs but no swelling of the feet nor petechia on the skin; put on sick list 7 November 1842, discharged 5 December 1842 cured.
Folios 14-15: case no 9, Charles Owen, aged 17, prisoner, taken ill at sea; sick or hurt, concussio cerebri, fell from the top of a cask on the iron part of a pump forward & appeared to have struck the right temple and side, he seemed uninjured, but after a few minutes fell down insensible; put on sick list 9 November 1842, discharged 17 November 1842 cured.
Folios 15-16: case no 10, James Rendall, aged 30, prisoner, taken ill at sea; sick or hurt, scorbutus, gums swollen of a dark livid colour and separated from the teeth, gums of lower jaw chiefly affected; put on sick list 10 November 1842, discharged 4 December 1842 cured.
Folios 16-17: case no 11, William Langhurst, aged 50, prisoner, taken ill at sea; sick or hurt, phthisis pulmonalis, a feeble worn out subject, complained of fixed pain in left side aggravated by inspiration or motion of the ribs, had cough and dyspnoea with purulent expectoration; put on sick list 1 November 1842, died 19 November 1842 at 5 am.
Folios 17-19: case no 12, William Carroll, aged 21, prisoner, taken ill at sea; sick or hurt, pleuritis scorbutus, was seized during the night with pain of left side of chest between the 7th and 8th ribs, breathed very difficult and cannot inflate the chest without a great pain, short dry cough and a quick but not strong pulse; put on sick list 15 November 1842, sent 6 December 1842 to Hospital at Hobart Town.
Folios 19-20A: case no 13, Joseph Maltby, aged 22, prisoner, taken ill at sea; sick or hurt, scorbutus, an incipient symptoms, upper edge of gums livid legs and thighs covered with small livid spots, pain in left knee; put on sick list 20 November 1842, sent 6 December 1842 to Hospital at Hobart Town.
Folios 20A-21: case no 14, James Little, aged 20, prisoner, taken ill at sea; sick or hurt, scorbutus, of a thin spare habit of body, gums livid and painful, numerous small livid spots on legs, encircling the roots of the hairs, pain near the right tendo achilles, bowels regular, no apparent debility; put on sick list 17 November 1842, sent 6 December 1842 to Hospital at Hobart Town.
Folios 21-22: case no 15, George Brown, aged 20, prisoner, taken ill at sea; sick or hurt, scorbutus, pain and stiffness in ham string extended down the legs, was very weak, complexion sallow, appetite impaired, gums began to look livid and spongy; put on sick list 22 November 1842, sent 6 December 1842 to Hospital at Hobart Town.
Folio 22: case no 16, William Frost, aged 19, prisoner, taken ill at sea; sick or hurt, scorbutus, fungous state of gums chiefly about the molares, appetite good, no petechia or other scorbutic symptom; put on sick list 21 November 1842, discharged 4 December 1842 cured.
Folio 22: case no 17, William Cousins, aged 27, prisoner, taken ill at sea; sick or hurt, scorbutus, gums livid and spongy with occasional haemorrhage no other symptom of scurvy; put on sick list 22 November 1842, discharged 4 December 1842 cured.
Folio 23: case no 18, Edward Jackson, aged 27, prisoner, taken ill at Hobart Town; sick or hurt, pleuritis, a stout man, was in a good health all the voyage, complained of acute pain in right side between 5th and 6th ribs the pain is confined to a small spot increased on pressure or inspiratory movement; put on sick list 3 December 1842, sent 6 December 1842 to Hospital at Hobart Town.

Folios 29-32: Surgeon's general remarks. A guard consisting of forty soldiers of the 99th Regiment with four women and four children joined the ship at Deptford on 21st July 1842. On the 30th, 200 male convicts were received from the hulks at Portsmouth, on the 2nd August the ship arrived at Plymouth Sound and the following day 190 convicts were embarked from the Stirling Castle hulk making the total of 390 prisoners. The convicts were generally young men and seemed in a good health. According to the surgeon, every assistance was afforded by the surgeon of the hulks in the selection of men fit for the voyage, despite the vigilance of the medical officers some improper persons found their way on board, effectually concealing their ailment. In this regard the surgeon stated that two cases of confirmed phthisis [case nos 2 & 11] were detected soon after leaving England which terminated fatally, one in an advanced stage of the disease when he came on board the other had likewise been ailing in England and hastened by the privation of a long sea voyage. The first case of scurvy was detected on the 7th November [case no 8] and the second case on the 10th, in a week afterwards the whole number of scorbutic cases amounted to eleven. In reference to the lime juice, citric acid and nitrate of potass in the cure of scurvy, the surgeon stated that he was not prepared to give a decided opinion, as a result of too few in the number of the cases and more especially the short time they were under treatment, however he was intended, if circumstances allowed to try the effect of improved diet alone in checking the progress of scurvy, but as mentioned the disease only appeared in the last week of the voyage and no time was afforded for trying any other method of treatment in a satisfactory manner.
