Folio 43: Case no 113 [115], Michael Fitzgerald, aged 21, ordinary seaman; taken ill at Cadiz Bay; disease or hurt, severe wound and bruise of the left leg by falling from the hammock; taken ill, 18 May 1812; invalided 6 August 1812 to hospital.

Folios 43-44: Case no 114 [116], James Burk, aged 24, ordinary seaman; taken ill at Cadiz Bay; disease or hurt, lues venerea; taken ill, 29 June 1812; discharged 29 August 1812 to duty.

Folio 44: Case no 115 [117], John McCarty, aged 28, ordinary seaman; taken ill at Cadiz Bay; disease or hurt, lues venerea; taken ill, 7 July 1812; discharged 23 August 1812 convalescent.

Folio 44: Case no 116 [118], Richard Brown, aged 30, quarter gunner; taken ill at Cadiz Bay; disease or hurt, ulcer; taken ill, 27 June 1812; discharged 3 August 1812 to duty.

Folios 45-46: Case no 117 [119], Christopher Golding, aged 40; taken ill at Cadiz Bay; disease or hurt, fever; taken ill, 5 July 1812; discharged 26 July 1812 to duty.

Folio 46: Case no 118 [120], David Brown, aged 26, able seaman; taken ill on passage to England; disease or hurt, stricture; taken ill, 20 July 1812; sent 13 August 1812 to hospital.

Folio 46: Case no 119 [121], Michael Donaldson, aged 30, ordinary seaman; taken ill at Cadiz Bay; disease or hurt, servere wound and bruise of the fore finger of the left hand; taken ill, 11 July 1812; discharged 29 July 1812 to duty.

Folio 46: Case no 120 [122], John Neil, aged 32, able seaman; taken ill at Channel; disease or hurt, accident, contused wound of the right hand; taken ill, 1 August 1812; invalided 31 August 1812 to go on shore.

Folio 49: Surgeon's general remarks and numerical abstract of the cases listed in the journal. Remark on insufficient supplied of fresh meat and vegetable which possible the coursed of scurvy.
